I understand that the Indian Hills Scholarship Committee will review my application and determine whether I am approved for a scholarship. Then the Indian Hills Scholarship Coordinator and others will determine the scholarship for which I am eligible based upon the information included in and mailed for this application.
I agree that should I be awarded an Indian Hills Foundation Scholarship, I will supply the Foundation with my photograph and biographical information for use in Foundation publicity immediately upon request.
I understand that certain directory information (name, address, telephone listing, email address, field of study, previous schools attended, photographs, date and place of birth, participation in officially recognized activities and sports, dates of attendance, degrees and awards, full- or part-time status, expected date of graduation, and weight and height of athletes) about me could be provided to a donor if I am considered for a designated scholarship.
If I do not maintain satisfactory progress, I may be ineligible to receive the next term or next year award unless I submit an appeal letter and receive approval for the award to be reinstated.
If I have not already done so, I will submit my high school transcript or GED scores to the Indian Hills Admissions Office (623 Indian Hills Drive, Ottumwa, IA 52501) by March 15 for first priority consideration.
A general letter of reference is to be forwarded to the Indian Hills Foundation at 525 Grandview Ave., Ottumwa, IA 52501 or emailed by the person providing the reference to Becky.Henderson@indianhills.edu . The letter should be written by someone who knows me well (teacher, employer, co-worker, etc.) but is not a relative. The letter must include my name, a brief summary of how the writer knows me, why I am a good candidate for a scholarship, and the name and title of the person submitting the reference.
The deadline for first priority consideration is March 15. Applications and materials received after this date and before June 15 will be reviewed at the Scholarship Committee’s next meeting in July.
